Job Description

**Responsibilities** + Work with Registered Nurse Manager to provide timely and efficient identification of patients who need discharge planning. + Provide assessments of psychosocial impact of illness. + Provide direct services including discharge planning for patients needing community resources prior to discharge from the acute care setting. + Experience in a medical setting is preferred. **_We are offering a sign on bonus up to $10,000 to hires who meet the eligibility requirements, also referral bonuses, excellent benefits, tuition reimbursement and relocation assistance._** **Qualifications** + A current unencumbered Social Worker license to practice in the State of Arkansas is required + Bachelor of Social Work degree + BLS within 30 days + Must be able to communicate effectively, both written and oral. + Must possess organizational skills, able to prioritize work. + Must possess demonstrated training, knowledge, and skills necessary/appropriate to the adult, and/or geriatric age groups **Overview** CHI St. Vincent a regional health network serving Arkansas is part of CommonSpirit Health.
