Responsibilities The Home Health Patient Care Manager supervises and coordinates clinical services, providing nursing care to patients at home as prescribed by physicians and in compliance with laws, regulations, and agency policies. Provides clinical services within the scope of practice, as defined by state laws, in accordance with the care plan and in coordination with the healthcare team Receives referrals, assigns clinicians, evaluates patient orders, and schedules start of care visits Coordinates patient home health benefits, medical necessity, and insurance approvals Assesses patient needs continually and ensures care is individualized, appropriate, and compliant with physician orders Reviews assessments and care plans daily, consulting clinicians as needed Education and Experience Current RN licensure in the state of practice Current CPR certification Valid driver's license, vehicle insurance, and access to reliable transportation Company Overview LHC Group is an Equal Employment Opportunity employer.
